Please fix affiliate signup page

I can't enter in details for payment option and it's preventing me from signing up to the program.

Please fix so I can signup.

Was about to post the exact same thing.

Specifically when selecting NETELLER the account number and Neteller email boxes don't appear, but they are required to finish the sign up.

I emailed FGP days ago about this but no reply.

I don't even think they have an active affiliate manager (if they even have one). The last one was Debee, so she may know more - but I don't recall a replacement.
