I'm not so sure but it can happen if you copy contents from one site to another with making good changes. I saw one of my friend facing this type of problem because of copying contents to his one site to another.

I do not have much idea about this but I guess same IP for same type of sites is a wrong idea but you may use same IP for different types of sites.

It actually depends on the purpose of your work. If it's for Google ranking and you are ready to maintain the regularity I guess one big site is better, because with lots of contents and separate tables you can give your site a nice shape that helps you to get good traffic as well as better...
