we have prepared a survey and we would like affiliates to have a look and reply. It wont take more than 5-10 minutes.

Our aim is to better understand the affiliates' needs and importance of things compared to others so we can better match the goals they have set.

Not to be picky, but you might get more responses if you make this:

2. Adobe PDF (writable Form)


I was going to fill it out, but I have no printer here and cannot check the boxes in Microsoft Word. Makes it kind of useless until I print it out.

you are right about the ticking, Word has an option to have boxes where with a click of a mouse they get ticked, however this format doesn't allow editing the open ended questions. So I added a note for affiliates, to delete the box and replace it with an X.

We already got some interesting replies and I plan to post some findings here for everyone to see once we are done with the analysis.
